The patient’s right to confidentiality
You have the right to confidentiality in the handling of your health information except when reporting is required by law. You have the right to personal privacy as written in the federal regulation, Health Insurance Portability and Accountability Act (HIPAA). We will make reasonable efforts to see that its uses, disclosures, and requests for disclosure of Protected Health Information (PHI) are limited to the minimum amount necessary to accomplish the billing and collection of your account(s).

Responsibilities of the patient
Your Responsibility to Provide Complete and Accurate Billing Information
It is the responsibility of the patient that we are provided with complete health insurance information upon check-in and prior to seeing the physician. This includes presenting a valid driver’s license or ID, and the most recent health insurance cards.

It is the responsibility of the patient to be knowledgeable of the information relating to your plan coverage at the time service is provided. Please understand and comply with the requirements of your insurance. Know your benefits, be aware of what services are covered or not covered and obtain proper certification if required prior to services being rendered. If you receive a service that is not covered by your insurance, you may be asked to sign an Advance Beneficiary Notice or Letter of Non-Coverage to signify that you have been informed of your payment responsibilities.

The Financial Responsibility of the Patient or Patient Guarantor
It is responsibility of the patient or patient guarantor to meet the financial obligation at time of service. This includes but is not limited to co-pay, co-insurance, or payment in full for services rendered if no insurance has been presented.

The patient or patient guarantor is expected to respond promptly to requests for information necessary for claims payment either from their insurance carrier or the physician’s office. Failure to do so may result in any outstanding balance becoming the responsibility of the patient or the patient guarantor.

Although Western Kentucky Kidney Specialists maintains an active and aggressive follow-up effort with all insurance carriers, it is the responsibility of the patient or patient guarantor to contact their insurance company to ensure prompt payment of their accounts.

A timely payment on the patient portion of the balance is expected. Payment of the balance is the responsibility of the guarantor. For your convenience we accept cash, check, MasterCard, Visa, Discover, and American Express.

Please let Western Kentucky Kidney Specialists know if you anticipate issues with paying your bill. The Patient Services Department is available for your convenience between the hours of 7 am and 4 pm, Monday through Friday.
